We are currently registering for the 2023-24 and 2024-25 school years. Please be mindful of selecting the correct year on this form.

Prior to completing this form, make sure you have ALL of the following documents ready to upload:

• Birth Certificate
• Immunization Records and Recent Physical Examination
• Unofficial Transcript and Report Card from prior school
• Course Recommendations for new school year
• Evidence of Dominicle
• Consent for Release of Records Form
• Three (3) utility bills reflecting student address in the district
• Reidential lease (if property is a rental) OR closing documents or property deed (if property is a purchase)

There are separate file size limits for Photos (.jpg) versus Document files (.doc or .pdf) as follows:

Image Files: 10 Mb | Documents: 3.5 Mb

This limit pertains to each individual file being uploaded. The total size of all the files may exceed the maximums above.

All registrations for Green Brook residents must take place at the Green Brook Board of Education first and then you may enter information here.
